Thank you for your interest in joining Paws-to-Share as make visits to schools in the Santa Clara Valley.  There are a number of forms to complete, so we've created on line versions that you can easily complete, without having to print and scan.  So please find the links here, and do please get get these filled out at your very earliest convenience.  Once this packet of forms is complete there will be some additional steps, including...

       •    Live Scan Appointment – directions with come from SCCOE directly
       •    Proof of TB clearance by either a TB Risk Assessment or a TB test 
       •    Providing a picture for SCCOE badge (clear and well-lit, from shoulders up)
       •    SCCOE required training  – again, directions with come from SCCOE directly
             -    Preventing Harassment and Discrimination  
             -    One-Time Mandated Reporter Training (General Training) 
             -    Mandated Reporter Training (School Personnel)
