Please complete the form below to order the stool test required to join join at Learn4Life Charter High Schools. As with all requirements for Paws-to-Share, there is no cost to you, and the fees for the test are covered by Paws-to-Share.  As a note, the results of this test will be shared with Paws-to-Share and Learn4Life, so please provide your approval at the end of this form to have those results shared.  If the test does show the presence of a parasite please seek treatment from your veterinarian, and once treatment is complete, please come back and order another test.
